NO LONGER A WALK IN SERVICE.

FOR PATIENTS REGISTERED WITH A GP IN HEYWOOD, MIDDLETON, ROCHDALE ( OUT OF THESE HOURS) PLEASE TELEPHONE 0161 763 8292 FOR AN APPOINTMENT.

Please note the opening times opposite. We are closed on Weekends and Bank holidays.

The Surgery will be closed on the 4th WEDNESDAY of each month for staff training. We will close at 1pm and will not open again until Thursday morning at 8.00am

Who We are

Middleton Health Centre, which opened in December 2009, is one of nine GP practices in Greater Manchester that are run by Hope Citadel Healthcare.

Hope Citadel Healthcare is a not-for-profit community interest company, who have been commissioned by local CCG’s to provide NHS services to the local population where the practices are located.
